Job Summary


Administrative Office Assistant maintains clinical charts and files in a timely manner.  Mails and tracks physician orders to ensure returned signed within the time allotted by state and federal regulations.  Performs other assigned clerical and administrative duties to support smooth operation of the agency.

Job Qualifications

 

  • High school diploma or equivalent
  • One year proven working knowledge of home health/hospice.
  • Excellent communication skills.  Basic typing and computer skills with knowledge of medical terminology.  Ability to use other office equipment.  Good organization skills with attention to detail.
  • Must be able to work both with others and independently.

Registered Nurse

Job Summary

The Home Health Registered Nurse provides skilled nursing care to patients on an intermittent basis in the home, independently assessing, planning, and delivering this care in accordance with the physician's orders and under the general supervision of the Director of Clinical Operations.  Acts as a liasion to ensure coordination of all disciplines in each case to ensure continuity of care for each patient.

Job Qualifications

  • Current North Carolina License for Registered Nurse.
  • Current CPR Certification.
  • Minimum of two years experience as a Registered Nurse, with six months experience in a Home Health setting.
  • Must have sound clinical knowledge, including a thorough understanding of nursing process and its application.
  • Has a high energy level, ability to use independent judgement and work with minimum supervision in a fast pace environment.
  • Has good communication skills both oral and written and has organizational ability necessary to manage multiple priorities.

Intake Assistant-Insurance Case Management

Job Summary

Responsible for answering telephone, verifying insurance, processing referrals, data entry and providing support to the Clinical team which includes the daily operations of the centralized intake functions within the department.  Also responsible for insurance case management duties such as obtaining authorization for services, requesting updates from appropriate disciplines, faxing and telephoning to request continued authorization and informing Clinical Managers.  Registered Nurse preferred.

Job Qualifications

  • Must have high school diploma or equivalent.
  • If Registered Nurse, must have current North Carolina RN license.
  • Knowledge of Home Health/Home Care Services to be of assistance to referral sources.
  • Knowledge of Medicare/Medicaid rules/regulations necessary to understand criteria for determining patient eligibility for admission.

Director of Clinical Outcomes

Job Summary

Participates in and provides leadership for Home Health Performance Improvement initiatives that include support for evaluation of clinical documentation, compliance oversight, and teams to evaluate processes, practices, and/or programs for improvement.

Job Qualifications

  • Current North Carolina license for Registered Nurse.
  • BSN or equivalent experience.
  • Three years of experience in Performance Improvement (QA) in a healthcare setting.
  • One year of management experience preferred.
  • Demonstrates ability to utilize Quality Improvement model tools.
  • Demonstrates thorough knowledge of Medicare guidelines and regulations, federal and state regulations, and ACHC standards.

Licensed Physical Therapy Assistant (LPTA)

Job Summary

Under the supervision of a Licensed/Registered Physical Therapist, provides Physical Therapy services to patients in their own homes or contracted institutions, under Physician's orders, to restore or improve physical functioning impaired by illness, accident or disease.  The LPTA participates in educational programs and performs appropriate non-patient care duties.  Flexible hours will be required to provide for late evening an dweekend demands on an as needed basis and each LPTA will be expected to accommodate these demands as they arise.

Job Qualifications

  • Must have current North Carolina Physical Therapy Assistant certification and current CPR.
  • At least one year clinical experience in a rehabilitation or acute care setting.
  • Complies with accepted professional standards and principles.
  • Has critical thinking skills, decisive judgement and the ability to work with minimum supervision in a fast pace environment.
  • Has good communication skills both oral and written and has organizational ability necessary to complete and submit all required documentation of patient care wihin established time frames.
  • Is flexible and cooperative in fulfilling role obligation.
