Job Summary

Responsible for providing direct care to patients on an intermittent basis in the home.  Assesses the needs for, and refers to, appropriate resources, as well as the basic health care needs of the client.  Acts as liaison among staff, patients, family, physicians, and other care providers.

Job Qualifications

  • Current North Carolina License for Licensed Practical Nurse.
  • Current CPR Certification.
  • Minimum of one-year experience as a Licensed Practical Nurse with six months experience in a Home Health setting.
  • Must have sound clinical knowledge, including a thorough understanding of nursing process and its application.
  • Ability to work independently with minimal supervision and effectively deal with stress.
  • Ability to deal tactfully with customers and the community.

Physical Therapist

Job Summary

Physical Therapist provides medically prescribed physical therapy services to patients in their own homes or contracted institutions, under Physician's orders, to restore or improve physical functioning impaired by illness, accident or disease.  The Physical Therapist participates in educational programs and performs appropriate non-patient care duties.  Flexible hours will be required to provide for late evening and weekend demands on an as needed basis and each Physical Therapist will be expected to accommodate these demands as they arise.

Job Qualifications

  • Must have current North Carolina license for Physical Therapy and current CPR.
  • At least two years clinical experience.  Clinical affiliations will be considered a portion of the experience requirement.
  • Complies with accepted professional standards and principles.
  • Has critical thinking skills, decisive judgement and the ability to work with minimum supervision in a fast pace environment.
  • Has good communication skills both oral and written and has organizational ability necessary to complete and submit all required documentation of patient care within established time frames.
  • Is flexible and cooperative in fulfilling role obligation.

Skilled Nursing Care

Our Registered Nurses provide Skilled Nursing on a part-time or intermittent basis and this care could include wound care (including wound vac. therapy), IV therapy, venipuncture, teaching regarding diagnosis and pain management as well as many other related services.

Home Health Aide Services

Our Home Health Aides are sent to the patient’s home on an intermittent basis to help with personal care such as bathing or dressing.

Physical Therapy

Our Physical Therapist provides exercise to regain movement, strength and the ability to perform self-care tasks as well as home safety management.

Speech Therapy

Our Speech Therapist provides exercises to regain speech and improve swallow function. Your Guardian Health Services professional can also help with training in the use of communication devices.

Occupational Therapy

Our Occupational Therapist provides exercises to regain the ability to perform self-care tasks, transfers, use adaptive equipment and regain strength.

Medical Social Work

Our Medical Social Worker helps the patient with social and emotional concerns related to their illness. Based on the patient’s individual care plan, this may include short-term counseling, community resource identification, and long-range planning.

Specialized Services

Wound Care Management

Guardian Health Services has a WCC (Wound Care Certified) nurse who serves as a resource to physicians and clinical staff by providing would care assessments and recommendations for patients with non-healing or problematic wounds.

In-House Wound VAC

Guardian Health Services has teamed with KCI to have Activac Portable Wound VAC systems in the office thereby providing prompt availability for patients. The VAC Therapy system has been clinically proven to help promote wound healing while helping decrease the incidence of complications.

Enterstomal Therapy

Enterstomal Therapy for Ostomy, stoma and wound care is provided by skilled nurses in the patient’s home.

Pulsed Lavage

Pulsed Lavage is a therapy that serves as an alternative to whirlpool therapy and is provided by a physical therapist in the patient’s home.

Certified Lymphedema Specialist

Guardian Health Services has a Physical Therapist who is certified to provide Lymphedema treatments through decongestive therapy, pressure wrapping and compression stocking management.

Pediatric Care

Guardian Health Services has experienced and qualified staff who understand the special medical needs of infants and children.

Telemonitoring

Telemonitoring systems provides qualified patients a sense of security knowing our clinicians are monitoring their health status 365 days a year. The monitor encourages self-management which can reduce preventable ER visits and hospitalizations.

Guardian Health Services Discharge to Monitor Program helps cardiac and respiratory patients discharged from the hospital manage their conditions more effectively thereby reducing 30-day hospital re-admission rates. By utilizing this program, hospitals will know the status of monitored patients without hospital re-admission.

Lifeline

Lifeline is an easy-to-use medical alert service designed to reduce the risk of living alone. In the event of a fall or emergency, you are connected to a trained Personal Response Associate – 24 hours a day, 365 days a year.
